There are two basic types of application packages submitted to the Local Agency Formation Commission. First is an application for a change of organization/reorganization (e.g. annexation, detachment, consolidation, formation/dissolution of a special district, incorporation/disincorporation of a city, activation or divestiture to provide a service, etc.) or a sphere of influence amendment. The second type is an application to request authorization to provide service by contract. Below you will be able to view the relevant application documents within each type of application package.

APPLICATION FOR EXTENSION OF SERVICE BY CONTRACT
OUT OF AGENCY SERVICE CONTRACT POLICIES AND PROCEDURES
If a city or district is proposing to provide a service or services outside its boundaries by contract or agreement, the Local Agency Formation Commission has adopted policies related to out of agency service contracts and established application and review procedures for requesting authorization to provide service by contract. If a fire service provider is proposing to provide fire protection service by contract, the Local Agency Formation Commission has adopted policies related to fire protection contracts and established application and review procedures for requesting approval of a fire protection contract.
